Fentanyl and carfentanil are potent opioid analgesics, posing a significant exposure risk to anyone encountering the substance. Dosages in the microgram (less than a grain of sand) range can be toxic. Exposure by inhalation of aerosolized powder, transdermal transportation, and accidental passage by hand to mouth are possible. The dosage necessary for a poisonous response is beyond the ability of the human eye to discern. Numerous accounts of accidental dosage resulting in a toxic response have been documented, and the threat of passive contamination is extreme. Fentanyl is, unfortunately, dangerous at levels that are not readily visible to the naked eye. Milligram quantities could easily be overlooked or not seen at all when dispersed over a mottled and articulated surface.
Emergency response personnel are subject to inadvertent exposure to contamination through direct patient contact or indirectly from environmental factors such as clandestine labs or drug encampments. First responders do not have readily available access to soap and water, which is the current protocol for personnel who believe that they may have come into contact with contamination.
Regardless, the use of sanitizing alcohol hand rubs is not recommended and exhibits no neutralization effect on fentanyl. The need for an effective and safe means to mitigate potential exposure is paramount,
